فرم ها


  • درخواست مجوزحضور میهمانان و هیات های خارجی در دانشگاه شیراز:
    به منظور ورود تمامی کارشناسان و اساتید خارجی به دانشگاه شیراز (شاملایرانیان مقیم خارج از کشور و شهروندان سایر ملل)، نیاز به کسب مجوز ورود به دانشگاه می­باشد. جهت انجام هماهنگی­های لازم، نیاز بی طی کردن مراحل زیر می­باشد:
  1. درخواست کتبی رئیس دانشکده به مدیریت مرکز همکاری های علمی و بین المللی دانشگاه شیراز مبنی بر دعوت از میهمان خارجی حداقل 3 هفته پیش از حضور میهمان. ذکر عنوان و زمان برنامه سخنرانی، نشست، کارگاه و ... در نامه ارسالی از دانشکده مربوطه ضروری می­باشد.
  2. ارسال تصویر خوانا از پاسپورت (و یا کارت ملی برای ایرانیان مقیم خارج از کشور) به پیوست نامه.
  • لازم به ذکر است ورود کارشناسان و اساتید به دانشگاه شیراز با ویزای توریستی ممنوع می­باشد مگر با صدور مجوزهای لازم از طریق مدیریت روابط بین­ الملل دانشگاه.

 

  • روند اخذ روادید جهت حضور میهمانان و هیات ­های خارجی:

جهت درخواست صدور روادید برای کارشناسان و متخصصان خارجی (شامل شهروندان سایر ملل) به منظور دعوت برای همکاری‌های علمی از جمله ایراد سخنرانی، بازدید،‌ مذاکره، شرکت در همایش‌های علمی بین‌المللی و غیره، لازم است مدارک ذیل حداقل ۴۰ روز قبل از تاریخ سفر میهمان خارجی ( برای میهمانان آمریکایی حداقل 60 روز) به دفتر روابط بین­ الملل دانشگاه ارائه گردد.

مدارک لازم:

درخواست کتبی رئیس دانشکده به همراه:

  1. فرم تکمیل و امضاء شده شماره یک مربوط به دعوت از میهمانان خارجی (فرم شماره ۱) (برای میهمانان آمریکایی، فایل ضمیمه نیز بایستی تکمیل گردد)
  2.  اسکن و تصویر گذرنامه میهمانان خارجی (فقط صفحه مشخصات) با فرمت JPEG و با حجم حداکثر ۳۵ کیلو بایت
  3. تکمیل و ارسال فایل word مشخصات میهمانان خارجی (فرم شماره ۲) 
  4. تکمیل و امضاء فرم همایش (فرم شماره ۳) توسط دبیر همایش و ارائه به دفتر با فرمت JPEG و حجم کمتر از ۳۵ کیلو بایت ارائه گردد.
  • برای میهمانان شهروند کشور آمریکا، علاوه بر فرم­های ذکر شده، فرم مخصوص میهمانان آمریکایی و فرم مخصوص میزبان بایستی تکمیل گردد.
  • لازم به ذکر است علاوه بر ارسال کلیه موارد ذکرشده به صورت کاغذی به دفتر روابط بین­ الملل دانشگاه، می­بایست هر کدام از فرم­ها (شماره 1، 2 و فرم همایش) به صورت یک فایل­ اسکن شده مجزا با فرمت PDF و با نام خانوادگی میهمان در یک ایمیل به روابط بین­ الملل دانشگاه (International@shirazu.ac.ir) ارسال گردد.
  • برای میهمانان خارجی شامل ایرانیان مقیم خارج از کشور نیازی به اخذ ویزا نمی­باشد.
  • علاوه بر دریافت روادید برای میهمانان خارجی، نیاز به دریافت مجوزحضور میهمانان و هیات­های خارجی در دانشگاه شیراز طبق فرایند قبل نیز می­باشد.
