فرم صلاحیت عمومی ( تکمیل این فرم برای پذیرفته شدگان از طریق آیین نامه ممتازین الزامی می باشد ، لازم است  این فرم و مدارک مورد نظر برای صلاحیت عمومی  که در متن اطلاعیه ، درج شده است تا آخر مهرماه 1400 به مدیریت تحصیلات تکمیلی دانشگاه تحویل داده شود و یا به آدرس این مدیریت :    شیراز - بلوار جمهوری - ساختمان مدیریت دانشگاه شیراز - طبقه سوم - مدیریت تحصیلات تکمیلی دانشگاه شیراز   کدپستی :  7194684471     ارسال گردد. )
